  1. Coordinated quarterly Child and Family Team Meetings.

    1. The child is expected to be a required participant.

    2. Parent/caregivers are still a part of the child and family team and should be invited if appropriate and safe for the child.

  2. Follow the Safety Framework Practice model to the fidelity in which ongoing engagement with parents/caregivers remains at the forefront of the custodians’ work to ensure family connections.

    1. Completion of PCFA/PCPA processes

    2. Ongoing monthly contact with child's parent/caregivers

    3. Monthly face to face contact with the child.

    4. Ongoing assessment of Impending Danger.

  3. Continue to complete relative searches at a minimum of quarterly to always be assessing placement with family.

  4. Engage in yearly permanency hearings to ensure APPLA continues to be the most appropriate goal for the child.

  5. Coordinate and facilitate sibling connections whenever possible.

  6. Coordinate and offer family interaction opportunities whenever possible.

  7. Complete IL referral and notify IL of APPLA goal.

  8. Review appropriateness of APPLA goal quarterly.
